How to Signup to become a Team Parent!

TEAM PARENT MEETING

* Thursday, February 23, 2006
*  Location:  Carlsbad Library Auditorium on Dove (in Vons Shopping Center next to La Costa Theatres)
*  Time:  6pm
 
*  IMPORTANT:  To receive your uniforms, you must fax in a completed "Medical Release Form" to the LCYO Toll-Free fax line at (866) 729-3071.  Your coach should have already done this.   If you do not have a fax machine, you may bring a COPY of the form with you, and then the uniforms will be released to you.
